Waiver and Release of Liability

1. Acknowledgment of Risk

I, the undersigned participant ("Participant"), acknowledge and understand that participation in training activities, including but not limited to physical exercise, fitness training, sports instruction, and related activities (collectively, "Training Activities"), involves inherent risks, dangers, and hazards that may result in serious personal injury, property damage, or even death.

I understand that these risks include, but are not limited to:

• Physical injury, including sprains, strains, fractures, and other bodily harm
• Cardiovascular events, including heart attack, stroke, or other medical emergencies
• Equipment malfunction or failure
• Slips, trips, falls, and other accidents
• Inadequate instruction or supervision
• Pre-existing medical conditions that may be aggravated by participation
• Environmental hazards and conditions
• Actions or negligence of other participants

2. Assumption of Risk

I voluntarily and knowingly assume all risks associated with my participation in Training Activities, whether known or unknown, foreseeable or unforeseeable, including those risks described above and any other risks that may arise from my participation.

I understand that no amount of care, caution, instruction, or expertise can eliminate these risks, and I choose to participate despite these risks.

3. Release and Waiver of Claims

In consideration of being permitted to participate in Training Activities, I hereby release, waive, discharge, and covenant not to sue the training provider, instructors, staff, agents, representatives, and all other persons or entities associated with the Training Activities (collectively, "Released Parties") from any and all claims, demands, actions, causes of action, suits, damages, losses, costs, expenses, and liabilities of any kind or nature whatsoever, whether known or unknown, suspected or unsuspected, arising out of or in any way connected with my participation in Training Activities.

This release includes, but is not limited to, claims for:

• Personal injury, including death
• Property damage or loss
• Medical expenses and costs
• Lost wages or income
• Pain and suffering
• Emotional distress
• Disability or disfigurement
• Any other damages or losses

4. Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, actions, causes of action, suits, damages, losses, costs, expenses, and liabilities (including reasonable attorneys' fees) arising out of or in any way connected with:

• My participation in Training Activities
• My violation of any rules, regulations, or instructions
• My negligent or intentional acts or omissions
• Any injury or damage caused by me to other participants or third parties

5. Medical Clearance and Health Representation

I represent and warrant that:

• I am in good physical condition and have no medical conditions that would prevent me from safely participating in Training Activities
• I have consulted with a qualified healthcare provider regarding my ability to participate in Training Activities
• I will immediately notify the training provider of any changes in my health status or medical condition
• I will not participate in Training Activities if I am under the influence of alcohol, drugs, or any other substances that may impair my judgment or physical abilities

Important: If you have any medical conditions, physical limitations, or concerns about your ability to participate safely, you must consult with a qualified healthcare provider before participating in any Training Activities.

6. Emergency Medical Treatment

In the event of an emergency, I consent to receive emergency medical treatment, including but not limited to first aid, CPR, and transportation to a medical facility. I understand that the Released Parties are not obligated to provide medical treatment and that any medical treatment provided is not a substitute for professional medical care.

I agree to be responsible for all costs associated with any medical treatment I may receive as a result of my participation in Training Activities.

7. Compliance with Rules and Instructions

I agree to:

• Follow all rules, regulations, and instructions provided by the training provider and instructors
• Use equipment properly and only as intended
• Report any unsafe conditions or equipment immediately
• Respect other participants and maintain appropriate conduct
• Stop participating immediately if I feel unwell or experience any pain or discomfort

8. Photography and Media Release

I consent to the use of my name, likeness, image, voice, and any photographs or video recordings taken during Training Activities for promotional, educational, or other purposes by the training provider, without compensation to me.

I understand that I may withdraw this consent at any time by providing written notice to the training provider.

9. Severability

If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect. The unenforceable or invalid provision shall be modified to the minimum extent necessary to make it enforceable and valid.

10. Governing Law

This waiver shall be governed by and construed in accordance with the laws of [Your Jurisdiction], without regard to conflict of law principles. Any legal action arising out of or relating to this waiver shall be brought in the courts of [Your Jurisdiction].

11. Entire Agreement

This waiver constitutes the entire agreement between the parties regarding the subject matter hereof and supersedes all prior or contemporaneous agreements, understandings, or communications, whether oral or written.

12. Acknowledgment of Understanding

I acknowledge that I have read this waiver in its entirety, understand its terms, and agree to be bound by them. I understand that this waiver is a legally binding document and that by participating in Training Activities, I am giving up important legal rights.

I further acknowledge that I have had the opportunity to consult with an attorney before signing this waiver and that I am signing it voluntarily and of my own free will.
